Illinois College Immunization Act Compliance for EIU International Students

Required Immunizations

The following proof of immunity is required:

  1. Diphtheria, Tetanus, Pertussis: Students shall provide dates of any combination of three or more doses of Diphtheria, Tetanus, and Pertussis containing vaccine.
    • One dose must be Tdap vaccine.
    • The last dose of vaccine (DPT, DTap, DT, Td, or Tdap) must have been received within 10 years prior to the term of current enrollment.
    • The minimum time interval between the first and second dose must have been at least four weeks (28 days), with the third dose having been received at least six months after the second or last dose of the basic series.
  2. Measles, Mumps, Rubella: Students shall provide documentation of receipt of two doses of MMR (Measles, Mumps Rubella) live vaccine.
    • The first dose must have been administered on or after the first birthday.
    • The minimum time interval between each dose must have been at least four weeks (28 days).
    • If shots records have been lost, positive titer testing results may be submitted only for MMR's.
  3. Meningococcal Vaccine: All new admissions  22 or under shall show proof of having at least one dose of meningococcal conjugate vaccine.
    1. This must be administered after the age of 16
    2. Must cover strains A&C, not meningitis B

 

Record Format

Records should be submitted using the World Health Organization's Official International Certificate of Vaccination, frequently referred to as that Carte Jaune or Yellow Card document,  from a clinic recognized and approved to provide and authenticate such records. 

Those unable to provide a WHO International Certificate of Vaccination, should provide original documents signed by primary care providers or other medical professionals.

Records are thoroughly reviewed, and questionable records are researched and investigated.  Submitting Fraudulent records may result in referral to the Dean of Students Office, judicial review, disciplinary action, including suspension and expulsion from the university.

Compliant immunization records MUST:

  • Be in English/transcribed in English
  • Be SIGNED and DATED by a health provider (MD, DO, APN, PA, NP, RN, LPN, MA, Pharmacist)
  • Must include the provider's/organization's complete address, as assigned by the government.
  • Must include a professional/business telephone number
  • Include the provider's licensure/registery information
  • Include the provider's/organization's professional/business e-mail and/or URL/webaddress

Documents that are NOT LEGIBLE or are INCOMPLETE WILL be sent back to the student.  We DO NOT keep incomplete records.

Failure to have verified immunization records by the end of the 2nd week of the semester will result in a registration hold being placed on the student's account, preventing them from registering for future classes.  Failure to have verified immunization records by the end of the 4th week of the semester will result in a $35 non-compliance charge.
