Student Insurance Enrollment and Waiver Process

The Student Insurance Fee is automatically included on the tuition and fee statement for students enrolled in an on-campus program with at least 9 hours. The cost for Student Insurance is $109.95 per semester for Fall/Spring and $87.45 for Summer. Student Insurance covers you anywhere off campus, here in the local community, at home, or anywhere you may travel, we do not have a network and provide coverage worldwide. Student Insurance helps pay the deductibles, copays and coinsurance left by your primary. Many students are out of network or have limited coverage when they are away from home.

Student Insurance Enrollment Form - Students are eligible to enroll in Fall 2023 Student Insurance if they are registered for 5 or more hours (any program format) on Census Day (September 1, 2023). Deadline to enroll is September 1, 2023.

Students wishing to purchase coverage must complete the Fall 2023 Student Insurance Enrollment Form by the applicable deadline.

Student Insurance Waiver Form - If you wish to waive the coverage, you will need to complete the Fall 2023 Student Insurance Waiver Form by September 1, 2023. This is for students billed for Fall 2023 Student Insurance. This includes students enrolled in an on-campus program for 9 or more hours for Fall, graduate assistants, or international students enrolled in 3 or more hours.

To access the foms, please follow the following instructions:

1. The student must complete the form. Parents/Others will not be able to access the form.
2. We recommend you use Chrome as your browser.
3. Log in to your Microsoft Account – portal.office.com using your EIU user name/password
4. Navigate to the EIU website
5. Access PAWS. The student must be logged in to PAWS with their EIU credentials (user name and password) to complete the Forms. If you are unable to remember your credentials or have trouble accessing the forms, please contact the EIU Help Desk at 217-581-5000. They will be happy to assist you.
6. Click on the MyHealth tab. This is where you will find the Student Insurance Forms and Instructions.
7. The deadline to enroll/purchase Student Insurance coverage for Fall 2023 is September 1, 2023.
8. The deadline to waive Student Insurance for Fall 2023 is September 1, 2023.
