Bias Incident Reporting Form

A bias incident or hate crime is any act of hostility motived by sexual orientation, gender identity, gender express, or any other minoritized community or status. These incidents can be reported through the Office of Student Standards, either anonymously or named. You do not need to know all the information, just put in as much information as you can.

If you have experienced a sexual assault or interpersonal violence you may complete the Sexual Assault/Interpersonal Violence form through the link below.
**Reporting through this channel will notify the Title IX Coordinator, Dr. Shawn Peoples, and the Deputy Title IX Coordinator, Dr. Heather Webb, and they will be required to investigate the incident per state law.

Individuals wishing not to report through a mandated reporter, may contact Lindsay Wilson, a Confidential Advisor at the EIU Counselling Center via phone (217) 581-3413 or email**

Click on the link below to access the reporting forms. You will be taken to the Office of Student Standards page, so scroll down on the page to view your reporting form.
