Bias Incident Reporting Form

A bias incident or hate crime is any act of hostility motived by sexual orientation, gender identity, gender expression, or any other minoritized community or status. These incidents can be reported through the Dean of Students Office, either anonymously or named. You do not need to know all the information, just put in as much information as you can. 

If you have experienced a sexual assault or interpersonal violence you may complete the Sexual Assault/Interpersonal Violence form through the link below.
**Reporting through this channel will notify the Title IX Coordinator, Dr. Shawn Peoples, and the Deputy Title IX Coordinator, and they will be required to investigate the incident per state law.

Individuals wishing not to report through a mandated reporter, may contact Jessica Cobert, a Confidential Advisor at the EIU Counselling Center via phone (217) 581-3413 or email jacobert@eiu.edu.**
