Thank you for enrolling in MySHIELDIllinois. Please follow the instructions below to securely access your online patient portal. MySHIELDIllinois allows you to view your test results and schedule/view past appointments.
How Do I Sign Up?
In your Internet browser, go to (https://my.shield.uillinois.edu).
Click on the Sign Up Now link on the right side. You will see the New Member Sign Up page. Enter your MySHIELDIllinois Access Code exactly as it appears below. You will not need to use this code after you've completed the sign-up process. If you do not sign up before the expiration date, you must request a new code.

Enter your Zip Code (xxxxx) and Date of Birth (mm/dd/yyyy) as indicated and click Submit. You will be taken to the next sign-up page.
Create a MySHIELDIllinois ID. This will be your MySHIELDIllinois login ID and cannot be changed, so think of one that is secure and easy to remember.
Create a MySHIELDIllinois password. You can change your password at any time.
Enter your Password Reset Question and Answer. This can be used at a later time if you forget your password.
Enter your e-mail address. You will receive e-mail notification when new information is available in MySHIELDIllinois.
Click Sign Up. You can now view your information.
