Subject: ADA Hybrid Reasonable Accommodation Form
As we learned yesterday from Dr. Glassman’s memo, plans are maturing for employees to return to campus. As a part of those plans, it was announced there would be a process in order for employees at high risk to apply for an accommodation. That form has now been developed and is available for use.
If you are an employee considered at high risk according to CDC guidelines (https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-at-higher-risk.html), please go to this link https://www.eiu.edu/humanres/machform/view.php?id=10268 to begin the interactive ADA Hybrid process.
