Our records indicate that your immunization record is either incomplete or we do not have any records on file, and has not fulfilled the University and State requirements for immunization compliance. A hold will be placed on your account September 18. Please provide a copy of your immunization record to the Health and Counseling Services - Medical Clinic. Your documents must be signed and dated by a nurse or doctor. Please fax documents to 1-844-256-6402. If we have not received a compliant record by October 1, a $35 late fee will be assessed to your account.
