Campers and Parents: Please print, complete, SIGN, and mail copies of all four forms below.  

1. Audio/Visual Consent Form

This form requires three separate SIGNATURES: the releasor, and two witnesses.  Please complete all three SIGNATURES prior to mailing.

2. Medical Treatment Authorization

Please complete and SIGN all fields prior to mailing.  You do not need to return the 2nd page, that is for your information only.

3. Over-the-Counter (OTC) & Emergency Treatment Authorization

Please complete and SIGN all fields prior to mailing.

4. Waiver of Liability

Please complete and SIGN all fields prior to mailing.
