Additional Information 

Employees and their dependents enrolled in any of the offered health plans are automatically provided with vision insurance at no cost.

Employees are automatically provided with Basic Term Life insurance coverage at no cost. The coverage is equal to the employee's annual salary. Employees can be insured for additional coverage through Member Optional Life. Additional coverage can be selected up to eight times the Basic Term Life amount. Employees can also choose Accidental Death and Dismemberment, Spouse Life, and Child Life insurance.

All benefit selections are made through CMS.
