Instructions for Liability Waiver Form

The liability waiver form should be used for students participating in University-related or sponsored activities that pose a risk of injury to the student. The release should not be used when the activity is required by a credit course, the activity will be considered in assigning a grade, and there is no acceptable substitute for the activity that would entail less risk to the student. The purpose of the release is to minimize the University's liability in the event a student is injured. The activities include field trips, off-campus activities, and extracurricular activities. You must fill in the blanks before the release is signed.

Please contact our office if you need to vary the language of the release, if you are not able to duplicate this form, or if you have other questions. The form is in PDF format, and you can complete the blanks using the tab key.

The completed and executed Release of Liability form should be kept in the files of the
faculty/administrator/staff member who is in charge of the activity for the length of time specified in your records retention schedule but for no less than five years. If an accident or injury to a student should occur during the activity, then the signed form should be provided to this office.
