Camp Registration Forms

All of these forms must be printed, filled out, signed, and returned to the School of Extended Learning/Academy of Lifelong Learning before minor campers will be officially admitted:

No camper will be admitted unless all forms are received before the first day of camp. Please mail forms with enough time for them to be received and processed.
