NOTICE: There have been recent changes regarding the Workers' Compensation procedures for incidents only where no treatment is necessary. The employee will notify their supervisor and the supervisor will report the injury to the Workers' Compensation Coordinator using an on-line form, Report for Incident Only. No further action is required unless treatment is later deemed necessary. If treatment is necessary, please see the procedures below for when treatment is necessary.
Procedures
Incident only - treatment is not necessary:
Once an employee has been injured on the job or suffers from a work-related injury/illness/disease, he/she should notify his/her immediate supervisor of the injury or illness.
The supervisor completes the on-line form, Report for Incident Only, with the employee (no treatment is necessary).
If treatment is necessary:
Once an employee has been injured on the job or suffers from a work-related injury/illness/disease, he/she should notify his/her immediate supervisor of the injury or illness.
The employee must contact the CareSys Early Intervention Hotline at 1-800-773-3221 or 1-800-526-0844 (TDD). If the employee is unable to make this call, the supervisor may place the call.
The next step is to complete the appropriate Workers' Compensation forms:
The forms are available on-line. Please contact the Workers' Compensation Coordinator at 217/581-7835 with any questions.
Return all completed forms to the Workers' Compensation Coordinator in Room 2010 Old Main.
If the forms are incomplete or incorrectly filled out, the Workers' Compensation Coordinator will return the forms along with a memorandum outlining what needs to be completed or corrected. This action may also involve calling the CareSys Early Intervention Hotline if it has not been done so yet (if treatment was necessary). Failure to complete any necessary actions could delay the processing of the claim and payment of bills.
Claims are processed and sent to Central Management Services Risk Management Division for review by a Claims Adjustor who will determine if the claim is compensable or non-compensable. Please note that this decision is NOT made at Eastern Illinois University by the Workers' Compensation Coordinator.
If the employee sought medical attention for the injury/illness, all medical bills and associated bills should be sent directly to:

Failure to inform the medical provider that the injury/illness is work-related will result in the employee being billed directly for the services or his/her insurance plan.
If the employee did not seek medical attention, see the procedure for incident only where no treatment is necessary.
