Instructions: Employees requesting leave related to the COVID-19 EMERGENCY PAID SICK LEAVE ACT should complete this form if you are requesting leave for one of the reasons listed below. Return this form by clicking “Submit” below, by email to benefits@eiu.edu or by fax to 217-581-3614. A Human Resource Representative will respond to the request within two (2) University business days.


If you have tested positive for COVID-19, you will be asked to provide a copy of the letter you receive from your local Health Department or a physician.


Do not report to work if you have been diagnosed with COVID-19, are exhibiting any symptoms of COVID-19, or if you have been in direct contact with an individual with a confirmed case of COVID-19. Please visit EIU's COVID Hub for updates.
