Individualized Study Courses Approval

Use this form to request enrollment in managed courses. It only takes a minute to fill out. You can now enroll a student in multiple semesters & courses on the same form.

One student per form.

If you are a student, meet with your professor to arrange the course and number of credit hours. The office will enroll you in the class.

This form must be submitted no later than 2pm on Census Day/Count Day. All approved requests will be completed by the close of business on Census Day/Count Day.
