Student Affairs - Leave Report Form
This form is for all A&P staff within the Student Affairs area, as well as the Civil Service exempt staff who are direct reports to the Vice President for Student Affairs. Please file this form whenever you are absent for one-half day or longer due to illness or vacation leave. All requests for vacation leave shall be submitted in advance to the Vice President for Student Affairs. Vacation leave shall be earned before taken.
Regarding absences due to illness:
Employees are individually responsible for notifying the Vice President for Student Affairs as soon as possible, but no later than the start of the assigned work day, if they will not be on duty because of illness. Messages can be left on the VPSA Office voice mail (581-3221).
Regarding absences due to vacation leave:
Administrative employees shall earn vacation leave at the rate of two days per month during each month or major fraction thereof, of service in pay status. Vacation leave may be accrued during the term of employment up to a maximum of 48 days. As administrative employee who accrues the maximum will earn no further vacation leave until the employee's use of vacation leave reduces the accrual to below the maximum. Supervisory approval is required prior to use of vacation leave.
Civil Service exempt employees shall earn vacation leave at the following rates:
0-3 years of service earns 25 days of vacation leave per year with a maximum of 50 days
3-6 years of service earns 26 days of vacation leave per year with a maximum of 52 days
6-9 years of service earns 27 days of vacation leave per year with a maximum of 54 days
9 + years of service earns 28 days of vacation leave per year with a maximum of 56 days
