Applicants to
Material to be Provided to Pre-Medical Advisor
1. A completed Medical Professions Student Information Sheet should be given to faculty members or others who are writing letters of evaluation. One copy must be provided to the Pre-Medical Advisor.
2. List of individuals who will be providing Letters of Evaluation for the applicant.
These letters should be sent directly to:
Ms. Pamela LeVine, Chair
Medical Professions Committee
Eastern
A composite letter will be written, and copies of the individual letters will be included in the package sent to the medical schools to which the student is applying.
3. List of Medical Schools to which the student is applying, with contact names and addresses.
4. Signed Medical Professions Committee Reference Waiver Form.
5. AAMC Application number.
6. Applicants must meet with the Pre-Medical Advisor (or communicate by phone or e-mail if off campus) before a letter will be written.
7. A copy of the Personal Statement included in
the Medical School Application is helpful but not required.
