Schedule a Benefits Orientation

  • As a new employee, you must attend a benefits orientation meeting within 10 days of the first day of your contract.  Group Benefits Orientations are held every Tuesday morning at 9:00 a.m. in Room 2031 Old Main.  You can reserve your seat via phone (217-581-5825) or e-mail benefits@eiu.edu
  • If you do not enroll within your first 10 days, your insurance will default to the following options:
    • Single Coverage in Quality Care Health Plan
    • Quality Care Dental Plan
  • Your benefits are effective on the first day of your contract.

 

What to Bring to Your Benefits Meeting

1. If you are adding dependents to your health plans:

 

Spouse

Marriage certificate.

Natural Child through age 25

Birth certificate.

Stepchild through age 25

Birth certificate indicating your spouse is the child’s parent and a marriage certificate indicating the child’s parent is your spouse.

Adopted Child through age 25 

Adoption certificate stamped by the circuit clerk.

Adjudicated Child/Legal Guardianship through age 25

Court documentation signed by a judge.

Adult Veteran Child
(IRS/non-IRS)
through age 29

Birth certificate, Eligibility Certification Statement (CMS-138)* and documentation as indicated on the ‘Documentation Requirements’ page of the Eligibility Certification Statement.

Disabled

Birth certificate, Eligibility Certification Statement (CMS-138)* and documentation as indicated on the ‘Documentation Requirements’ page of the Eligibility Certification Statement.

Other (organ transplant recipient)

Birth certificate, Eligibility Certification Statement (CMS-138)* and documentation as indicated on the ‘Documentation Requirements’ page of the Eligibility Certification Statement.

Civil Union Partner (and children of civil union partner)

Civil Union Certificate.

* The Eligibility Certification Statement (CMS-138) and the Statement of Health application are available on the Central Management Services website at www.benefitschoice.il.gov.


2. If you (or a dependent) are covered under another medical plan; and will continue that coverage, bring a copy of the insurance card (including Medicare).

3. Birth Certificates of children, Birth dates of spouses, and Social Security Numbers of your dependents and/or beneficiaries.

Members have 10 calendar days from their initial employment date to make health, dental, vision and life insurance coverage selections.

All members, including part-time employees, who fail to make benefit elections within the 10-day initial enrollment period will automatically be enrolled in the CIGNA Quality Care Health Plan (QCHP) and the Quality Care Dental Plan (QCDP) with no dependent coverage. They will also be provided with basic life coverage.
