Important Benefits Announcements The Benefit Choice Enrollment Period is the annual election period normally held from May 1 to May 31 each year. All state group insurance members (full- and part-time) may elect changes to any benefit program during this period. The changes will be effective July 1. At any other time, a change may be made within 60 days of a qualified change in family status. (Examples of a qualified change in family status include a birth, adoption, change in marital status, death of spouse or dependent, or change in spouse employment status.) Members may make the following changes during the annual Benefit Choice Period: -- Change health plans -- Elect to participate, or not participate, in the dental plan -- Add or drop dependent coverage for health or dental -- Increase or decrease member Optional Life Insurance and member AD&D coverage (Statement of Health approval is required to increase member Optional Life coverage, but is not required to add AD&D coverage) For more information on making changes to benefits elections, please contact the Benefits Services at 581-5825. New Employees have 10 calendar days from the initial employment date to enroll in various options. Members with less than 100% appointment have the option to waiver out of insurance, but they MUST sign the waiver form within the 10-day period. All members 50% or greater will have life insurance provided by the State of Illinois equal to their annual salary, at no cost to the member. Members who fail to make the benefit elections within the 10-day period will automatically be enrolled in the Quality Care Health Plan and Quality Care Dental Plan, and will be provided with Basic Life Insurance coverage. New Members may enroll eligible dependents within 10 calendar days of the Members' effective date. If dependent coverage is not elected during these first 10 days, the member must wait until the next eligibility period to request dependent coverage.
