This survey is a tool for you to learn about your personal health. This survey will take you approximately 5 minutes to complete. Once you have answered all of the questions, a personalized feedback report will be delivered.

You must be between the ages of 18-30 to complete this survey. By completing this survey, you are saying that you are at least 18 years of age.
