POLICIES FOR
DOCUMENTATION OF VISUAL IMPAIRMENTS
The University is committed to providing reasonable accommodations in academic programs as required by Federal statutes to students disabled by visual impairments.
Students requesting accommodations for a visual impairment will be required to provide documentation of disability from a licensed physician. This documentation should include information on the etiology, evaluation, and prognosis of the condition.
Also included should be additional documentation that will provide information relevant to the provision of appropriate support services. This will include, but not be limited to:
1. Functional limitations of the condition, particularly as it regards how the disability may impact on the student in an academic environment.
2. Information on technology, assistive devices, and support services with which the student is familiar OR information regarding technology, assistive devices, and support services which may benefit the student in an academic setting.
3. Medications/medical procedures prescribed for the condition and possible side effects of same which may affect the student in an academic setting.
