Dietetic Internship applications are accepted
for both the April (February 15 postmark deadline) and November (September 25
postmark deadline) computer match dates**. Please use the following
checklist to assist in the Dietetic Internship application process and mail
your completed application to: Karla

Letter of Intent. Please include your interest in dietetics, specifically nutrition education, as well as your interest in our program, your strengths and areas for improvement, and your short- and long-term goals in the field of dietetics. |

ADA Declaration of Intent or Verification Statement. The Declaration of Intent states your intent to complete your undergraduate coursework while the Verification Statement is issued after the completion of your undergraduate coursework. If accepted into our program, you must supply an original copy of the Verification Statement. |

Official copy of GRE score certifying score results. |

Official college transcripts certifying current enrollment in an approved/accredited program or completion of approved program. |

Letters of recommendation (minimum of three) from foods and nutrition faculty(at least one reference), work supervisors (at least one reference), and/or other faculty in sealed, signed envelopes. |

Non-refundable application fee of $35 for the Dietetic Internship payable to Eastern Illinois University. Please note that this fee is separate from the Graduate School fee. |

of unmatched applicants will be destroyed within one (1) month of notification.
