SALARY DISTRIBUTION OPTION -- FACULTY AND ADMINISTRATIVE STAFF
Faculty and administrative staff in nine-, 10- or 11-month positions who wish to change their salary distribution plan must complete a Salary Distribution Option form found on the HR website. The Salary Distribution Option form must be returned to the Human Resources Employment Office prior to the first day of the employee's contract. If a form is not received for new employees, the plan will default to salary paid according to contract dates. The salary distribution option plan chosen at this time cannot be changed during the current year and will continue in future years unless the Employment Office is notified of a change. FAMILY MEDICAL LEAVE ACT (FMLA) BASICS FOR SUPERVISORS
Family Medical Leave Act (FMLA) entitles employees to take reasonable leave of up to 12 weeks per year for medical reasons, for the birth or adoption of a child and for the care of a child, spouse or parent who has a serious health condition.

It is your responsibility as the supervisor to inform your employees of the need to complete and submit paperwork to the Benefits Office when you discover an employee is going to be off work consecutively or intermittently for more than two weeks.

How do you know if someone is eligible? What is the process to follow? What forms to use?

Richard Enyard, Human Resources director, will provide the answers to these questions and more during two sessions (choose one): 9 to 10:30 a.m. Tuesday, July 23, or 9 to 10:30 a.m. Thursday, July 25. Both sessions are in Phipps Auditorium, Physical Science Building, Room 1205.

HEALTHLINK INSURANCE CARD ERROR
Due to the change in co-payment amounts for the FY2014 plan year, managed care plan members were all sent new health insurance ID cards.

It has come to our attention that HealthLink OAP inadvertently sent new health insurance ID cards to all individuals who were ever dependents in the past on the HealthLink OAP memberís coverage. HealthLink OAP is advising members to disregard and destroy the ID cards for any dependent who is no longer on their coverage.

FSA MIGRATION FROM FBMC TO WAGEWORKS
The State of Illinois Medical Care and Dependent Care Assistance Programs, currently administered by Fringe Benefits Management Company, moved to a new claim-processing system supported by WageWorks Inc., effective July 1, 2013.

Between July 1, 2013, and July 14, 2013, all current MCAP and DCAP accounts were to be transitioned to the new WageWorks system. This two-week period was a HOLD PERIOD, during which no FY2013 claims were processed. This hold period was needed to transfer the accurate account balances to WageWorks. All FY2013 claims not submitted prior to June 30 will be reimbursed AFTER the hold period.

Due to this transition, there will be several changes for which you need to be aware. Please read the provided FAQs (located here) very carefully.
