NEWLY REVISED FORM I-9 IS NOW REQUIRED.
USCIS announced the newly revised Employment Eligibility Verification form, Form I-9, with the revision date of "(Rev. 03/08/13)N" is required beginning March 8, 2013. Prior versions can no longer be used. The new form can be found on the USCIS website here.

A few of the changes include improved instructions, additional data fields and expansion of the layout from one to two pages.

Please keep in mind that newly hired employees must still complete and sign Section 1 of Form I-9 no later than the first day of employment. If you have any questions about the new Form I-9 and how to correctly complete it, please contact Carol Galey at 581-7655 or Cindy Maples at 581-2613.

STATE CHANGES WORKERS' COMPENSATION PROCESSING
Pursuant to Public Act 97-895, the administration of the State Employee Workers' Compensation Program is being transitioned to a third party administrator. Effective March 16, 2013, Tristar Risk Enterprise Management Inc. will be the new processor for workers' compensation claims. Tristar is an experienced third party administrator that provides claims administration services to corporate and governmental entities across the United States.

Also effective March 16, 2013, there is a new phone number for reporting all new reports of injury that may result in a workers' compensation claim. It is 1-855-495-1554.

Tristar will have offices located in Chicago and Springfield. Existing claims will be transitioned to Tristar in the coming months. Each employee who has a claim currently active will receive an individual letter in the next few months with information regarding their new claim contact. STATEMENT OF ECONOMIC INTERESTS
Each year, the Illinois Governmental Ethics Act requires certain employees to complete and file a Statement of Economic Interests with the Secretary of State. If you:
1. Are, or function as, the head of a department;
2. Have direct supervisory authority or responsibility over contracts of $5,000 or more;
3. Have supervisory responsibility for 20 or more employees; or
4. Have responsibility with respect to the procurement of goods or services. (have a P-card or Office Max authority for example.),
then you fall under this requirement.

In the next few days you can expect to receive, at your home address, a letter and form to complete from the Secretary of State. It is important that you complete this form and forward it to me, either through campus mail, in person, or via USPS at: Sandy Bowman, Ethics Officer, Eastern Illinois University, 600 Lincoln Ave., Charleston, IL 61920.

RETIREMENT EDUCATION SEMINARS
The State University Retirement System (SURS) continues to hold Retirement Education Seminars for members enrolled in the Traditional and Portable plans.
