Automobile Insurance:

What to do if you have an accident in a vehicle while on University business

For State-owned or State-controlled vehicles:

  1. Notify your supervisor immediately.
  2. Notify the Office of the Vice President for Business Affairs immediately at extension 2921.
  3. Complete an "SR1" form after the accident, including the drawing and signature.
    • A blank form will be in the glove compartment of University vehicles, or available from the Motor Pool or will be available from the police officer at the scene.
    • If you will be out of town for more than three days, please fax a completed form to Business Affairs at (217) 581-3290.
    • If there is no form available and the damage involved only a University vehicle with no personal injuries, obtain a form from the Motor Pool during working hours upon your return to campus.
  4. In the event of an accident where there is serious bodily injury or death of any person involved, notify your supervisor, who will notify the Vice President for Business Affairs or designee as quickly as possible.

For your personal vehicle:

  1. Notify your personal insurance carrier. Steps 2-4 are the same as above.

All drivers should have a valid Illinois driver's license.

We sincerely hope these procedures will not be necessary. If you do happen to be involved in an accident, please make reporting a priority: failure for the University to convey information about your accident within the seven-day period allotted by Central Management Services could result in denial of coverage.

Professional Liability for Employees/Students:

Enrollment in an internship or practicum course is required for a student to be covered for professional liability insurance. To obtain a certificate of coverage, call extension 2921 and provide the following information:

  • Student's Name
  • Advisor's Name
  • Course Name and Number
  • Certificate Holder's Name and Address

A certificate of coverage is not always required and will, therefore, be provided upon request.
