1. Complete all sections of the Bachelor of Arts in General Studies Degree Program, Undergraduate Application for
    Admission (two parts).

2. Enclose a $30.00 application fee payable to Eastern Illinois University.
    Note:  If you are a State of Illinois Civil Service employee, please include your fee waiver form from your Personnel/Human
    Resources Department.  If you are a former EIU student, no fee is required.

3. Mail the packet to: Director, Bachelor of Arts in General Studies Degree Program, Eastern Illinois University, 600 Lincoln Avenue,
    Charleston, IL 61920

4. Request OFFICIAL transcripts from all accredited colleges and universities you have previously attended.

5. Xerox copies of military forms may be submitted.

6. In order for us to serve you as efficiently as possible, we need the completed application form and all transcripts before
    preparing a preliminary evaluation

7. If you have not heard from us within three weeks, please call the office.
