NOTICE: This information is regarding the workers' compensation procedures for incidents only, where no treatment is necessary.

Once an employee has been injured on the job or suffers from a work-related injury/illness/disease, he/she should notify his/her immediate supervisor of the injury or illness.

1. The supervisor completes this online form with the employee if no treatment is necessary.

2. No further action is required unless treatment is later deemed necessary. If treatment is necessary, please see the procedures for Filing a Claim.
