Mark your calendars for Oct. 18! The 2012 free flu shot clinic will take place from 9 a.m. to 4 p.m. in the Bridge Lounge, MLK Jr. Union. CMS requires state employees to present their state health insurance card and Panther Card at time of service.

RETIREES are required to present their health insurance card and two types of ID. If the retiree has Medicare, they need only to bring their Medicare card. Spouses of state employees are not eligible for the free vaccine. However, for a $30 charge, spouses of employees may get their flu shots at these clinics.

Remember to wear clothing that allows access to the upper arm.

SURS will have retirement counselors on campus Thursday, Nov. 8. If you wish to speak to a SURS representative on this date, they are now accepting appointments. Please call 1-800-275-7877 to obtain the necessary forms for your appointment. Once the completed forms have been received by SURS, you will then be mailed a notice to contact them again at the above number to schedule your appointment.

Appointments will be assigned on a first-come, first-served basis. Returning the forms in a timely manner will help ensure an appointment time. These forms can be faxed to 217-378-9800 or mailed to the address that is provided with the forms.

To schedule an individual counseling session, you must be within four years of retirement eligibility:
- Age 55 with eight or more years of service;
- Age 62 with five or more years of service; or
- 30 or more years of qualifying service at any age.

See attached form for additional information.

An eligible employee is entitled to an appointment or estimate every 12 months.

Two Internal Governing Policies were recently revised with the approval of the president. Please take time to review these changes.

-- No. 6. Benefits -- Sick Leave has added a FMLA requirement.

-- No. 37. Medical Clearance After Absence Due Illness or Injury now requires A&P employees to submit information to the Department of Human Resources instead of the Office of the President.

SECA is a once-a-year (Sept. 7–Nov. 2, 2012) campaign that provides all employees the opportunity to donate to the charitable causes of their choice to enhance the quality of life for all.

What Can Your Dollar Do To Help?

$1 per week…
-- Sends a child with epilepsy to a morning of summer camp
-- Provides a chest X-ray

$2 per week…
-- Provides five platelet counts for a child with hemophilia (bleeding disorders)
-- Helps distribute a Wizdom kit to a child newly diagnosed with diabetes
-- Provides infant mortality awareness information to 100 new parents

$4 per week…
-- Covers the cost of a spinal tap, including physician’s fee
-- Provides a red blood cell transfusion

$8 per week…
-- Provides one manual wheelchair for a person with a neuromuscular disease
-- Provides a day of oxygen

$10 per week…
-- Provides one day of chemotherapy to a young adult with leukemia
-- Sends a child with sickle-cell disease to camp for a week

$25 per week
-- Provides the funds for a sick child to spend one day in ICU

One time contribution…
-- $1 provides an educational booklet for a person newly diagnosed with multiple sclerosis
-- $10 provides diabetes risk tests to 500 individuals
-- $50 enables a hypertensive kidney patient to purchase a blood pressure cuff
