The State University Retirement System (SURS) continues to hold Retirement Education Seminars for members enrolled in the Traditional and Portable plans. If you are interested in these informational meetings, there are still seats remaining for the following dates: March 19, April 30 and May 15. These seminars are being held in the Champaign office, located at 1901 Fox Drive. Any faculty member who is terminating from the university for any reason from May through August should check their campus mailbox for a Clearance Sheet during the month of April. The Clearance Sheet is required when leaving the university, so please carefully read and follow the instructions on the form to avoid the inconvenience of receiving a final paper check. If you have any questions about the exiting process, or do not receive a Clearance Sheet, please contact Carol Galey (7655) or Cindy Maples (2613).

Effective immediately, the FMLA (Family Medical Leave Act) process will now be handled by the Human Resources Benefit Services Office. This change will centralize the process and allow for consistency in forms and services.

The Family and Medical Leave Act (FMLA) provides an entitlement of up to 12 weeks of job-protected, unpaid leave during any 12-month period to eligible, covered employees for the following reasons: 1) birth and care of the eligible employee's child, or placement for adoption or foster care of a child with the employee; 2) care of an immediate family member (spouse, child, parent) who has a serious health condition; or 3) care of the employee's own serious health condition.

Human Resources will be organizing training opportunities for the campus to learn more about FMLA and the responsibility of individuals and departments to manage this benefit.

If you will be out for medical reasons for yourself or a qualified family member for longer than three days, please contact the Benefit Services Office to notify them of this absence. Someone in the office can assist you with eligibility.

If you are a supervisor, and you have someone in your area who is or will be out for medical reasons for themselves or a qualified family member, please contact the Benefit Services Office to notify them of this potential FMLA.

The Benefit Services Office will be closed on Thursday, April 25, so personnel can attend Central Management Service's Benefit Choice Seminar in Springfield. This seminar will provide valuable information about the changes that will affect your insurance and benefits for the upcoming May Benefits Choice period. These changes will take effect July 1. For assistance in our absence, please visit or call the Human Resources Office at 581-3514. The office will re-open at 8 a.m. Friday, April 26.

Just in time for the summer, the Benefit Services Office announces the release of the OnLine Flex Form. The OnLine Flex Form allows for anyone to submit a flex schedule electronically, which helps in signature approval, while also saving paper. Anyone who is required to submit a summer flex schedule can do so by following this link. Through the completion of this form, the form will then be routed to your supervisor for approval. Following supervisor approval, the form will be electronically transmitted to the Benefit Services Office for final approval. Email notifications will notify you and your supervisor of the status of the submitted form.

In addition, the OnLine Flex Form can also be used any time an employee needs to request a schedule change, including a schedule change due to classes or a needed temporary modification to your current work schedule.

If you have already submitted a paper flex schedule, we ask that you consider re-submitting your schedule via this medium. If you do not wish to re-submit, thank you for your consideration and we hope your experience with the system next year will be positive.
