Using the Survey of Computer Lab form, complete
items 1-16 below. You can do this electronically by opening the attached
MS Word template, entering the appropriate data, then printing and signing
the form.
- Commonly used name for the lab facility.
- Person completing this survey.
- Date of survey completion.
- Building and room location of the lab.
- Is an accurate schedule of the open hours of the lab posted where
all students can easily read it?
- Are the ATAC approved "Open Lab Evaluation Forms" readily
available for students to report on the current condition of the facility?
- State the total number of general-purpose microcomputers available
for student use in this lab facility that have general-use software
including word processing, spreadsheet and web browsers?
- State the number of printers available for student use in this facility?
- State the number of student workers and graduate assistants who work
in this lab and the total number of weekly hours they work as assistants
in the lab facility.
- Provide the names and titles of person(s) (not student workers) whose
job is to supervise the operations of this lab and assistant students
using the facility during non-class lab hours. State the number of hours
per week each person is present in the lab facility.
- State the total number of hours this lab is opened for use by all
students, regardless of discipline, Monday through Friday. This total
should not include any times when the lab is scheduled for classes,
seminars, or other reserved activities.
- State the total number of hours this lab is opened for use by all
students, regardless of discipline, on Saturday and Sunday. This total
should not include any times when the lab is scheduled for classes,
seminars, or other reserved activities.
- State the average number of hours per week this lab is occupied and
used for scheduled classes during the fall and spring semesters.
- Which of the four types of a lab, as defined in the document "Definitions/Policies
Regarding Computer Replacement in Open Laboratories", do you feel
best describes this facility?
- Is any equipment the labs supported by the technology fee used for
any workshops or any similar training offered in which participants
are charged a fee (other than normal on-campus student fees)? If the
answer to this question is "yes" provide a separate page that
explains the following.
- The nature and content of these workshops.
- The typical participant.
- The average annual fee collected.
- How these fees are disbursed.
- State the name(s) and title(s) of the fiscal agent(s) who has/have
operational responsibilities for the lab facility. This person will
typically be a Dean, Chair, or Director. If this responsibility is shared,
state so and provide the name and signature of the person at the next
higher administrative level.
Using the attached MS Excel spreadsheet, provide a list of lab machines
including EIU tag number and a brief description of the machine, e.g.
Gateway P5 400mhz. Please attach a hardcopy of this document to the survey
described above. We would also appreciate it if you would e-mail a copy
of the spreadsheet to John Henderson.
Provide a weekly schedule of this lab facility showing the times for
non-class, scheduled class times, and any other type of usage during the
2003 spring and fall semesters.
