am or have been enrolled at one or more post-secondary institutions since I was last in attendance at Eastern. I am aware that I must have a composite grade point average of at least a 2.00 on a 4.00 scale for all course attempted at institutions other than Eastern Illinois University.

Transcripts have been sent or will be sent no later than three weeks after the grading period if I'm currently enrolled elsewhere. I understand that if the transcripts are not received or show that I do not meet the condition listed above, then I am not re-admissible to Eastern and am subject to having my enrollment canceled immediately.
