Students borrowing equipment from the Office of Disability Services will be required to sign an "Equipment Check-Out Contract" form, which will be returned to the student upon receipt of equipment that was checked out by the student. This form will contain information on the type of equipment, serial number/EIU tag number, value of equipment, and the duration of the loan.
If the student will require the use of the equipment after the return date noted on the form, the student must return to ODS and amend the Equipment Check-Out Contract form.
If the equipment is 1) returned damaged by other than normal use or 2) not returned, the student will be held responsible. Failure to discharge this responsibility may result in a monetary hold that will be placed on the student's record. The amount will be billed to the student's account.
