POLICIES FOR DOCUMENTATION
OF PSYCHIATRIC DISABILITIES
The University is committed to providing reasonable accommodations in academic programs as required by Federal statutes to students with disabling psychiatric disabilities.
Students requesting accommodations for a psychiatric disability will be required to provide documentation of the disability from a qualified, licensed professional, i.e. psychiatrist, doctoral level psychologist or child psychologist, or clinical social worker. Such documentation should be on letterhead or on the Psychiatric Documentation Form downloadable from the Office of Disability Service’s website and it should contain the professional's license number. The documentation should include the following information:
1. Diagnosis of the disorder, including DSM-IV classification.*
2. Specific criteria of the disorder indicating severity and course.
3. A description of major symptoms of the disorder as currently experienced by the student.
4. A description of functional limitations and/or behavioral manifestations currently experienced by the student, particularly those which may have impact on the student in an academic environment.
5. Medications currently prescribed and possible side effects, particularly those which may have impact on the student in an academic environment.
6. Information should be current, i.e. within three years of the date the student requests assistance.
