POLICIES FOR DOCUMENTATION

OF PHYSICAL AND FUNCTIONAL DISABILITIES

 

Eastern Illinois University is committed to providing reasonable equal access to students with physical and functional conditions which cause disability, as disability is defined in current Federal statutes. The University specifies these disabilities to be those which have a medical basis, are congenital or acquired, and which are not covered under other categories of disabling conditions as defined by this Handbook. Examples of physical and functional disabilities include, but are not limited to, diabetes, cerebral palsy, epilepsy, cancer, mobility impairments, cardiac disease, back injury, Autoimmune Deficiency Disorder, or pulmonary disease.

 

Appropriate documentation of such conditions must be provided by the appropriate licensed professional. Documentation should be provided on professional letterhead (prescription blanks are not acceptable) or on the form supplied by The Office of Disability Services and will include:

 

1.    Diagnosis of the condition

 

2.    Functional limitations of the condition on major life activities of the student, especially those which are likely to have an effect on the student in an academic environment.

 

3.    Prescribed medications/medical procedures and the possible effects these may have on the student in an academic environment.

 

4.    Suggestions regarding reasonable accommodations, as supported by the diagnosis, to assist the student to function in an academic environment.
